Personal Injury and Worker's Comp Treatment
We are happy to help those who have been injured as a result of an automobile accident or work related injury get back to feeling better, moving better and living their best life. We can file claims for you to Med Pay with your insurance if you have an order for massage therapy. We also take third party or liens on an individual basis.
Your physician's order for massage therapy needs to include the following: Diagnosis codes for treatment, length of sessions (60min session or 90 minute if you have several areas that are effected), how many sessions per week and how many weeks of treatment. For back pain or whiplash, most physicians recommend 60min session once or twice per week for 6 treatments and then we re-evaluate to renew the referral/prescription. You can bring a paper copy of the prescription or have it faxed to us at 719-631-0735 so we can schedule treatments.
We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, the following policies are honored:
24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 24 hours advance notice you will be charged the 50% of your appointment. This amount must be paid prior to your next scheduled appointment.
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged for their “missed” appointment.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
